What’s Covered?

The Home Accessibility Benefit covers the cost and install of home accessibility durable medical equipment (DME) including:

  • Wheelchair Lifts
  • Stair Glides
  • Ceiling Lifts
  • Metal Accessibility Ramps

Who Can Submit a Request?

Requests for the PHW Home Accessibility Benefit can be submitted by you, a qualified DME provider, or your doctor.

What Do I Need to Submit?

Requests for the Home Accessibility Benefit should include:

  • A prescription from your doctor
  • Clinical documentation that outlines and supports your need
  • Proof that you own your home or an approval letter from your landlord or homeowner.

How Do I Request this Benefit?

Call PHW Participant Services at 1-844-626-6813 (TTY 711). We can assist in submitting your request. Your doctor or DME provider can also start the process by faxing a request to 1-844-307-0997.

What Happens When My Request is Approved?

Once approved, PHW will work with you and the DME provider to install your accessibility item. Your PHW representative will monitor the installation to completion. We are available after the install to address any problems that may arise.
