How to Enroll
Visit Pennsylvania Department of Human Services to apply for or renew your benefits. You may choose to apply online, with a paper applications, or in-person for Community HealthChoices.
You can also check your eligibility before submitting your application using the online COMPASS website.

Visit your local county assistance office and apply in person.

Download the paper application and mail it to your local county assistance office.

Co-Pays
There is a $3.00 co-pay for brand name prescription drugs for participants without Medicare coverage. There are no additional co-pays. PA Health & Wellness fully covers your medically necessary needs.
